COVID in People with Rare Conditions Survey

This survey is used to determine whether a rare condition predisposes a person to more or less severe COVID symptoms.

Please click on the link COVID in Communities to take the survey. It takes about 30 minutes and is offered in English and Spanish.

There is an optional follow-up survey that will ask similar questions in about 6 months.

At the end of the initial survey, it will ask if the individual with the rare condition lives with someone else. If yes, it will ask if they want to enroll. This second participant allows us to learn more about how other people in the home were affected.

NIH (National Institutes of Health) wants responses from people who have and have not had COVID, so don’t skip the survey even you have not been exposed!
